Case Manager (Manager for Care Planning and Practice)

About the Role
As Manager (Care Planning & Practice) , you will lead a team of Support Workers caring for clients across Disability and Child Safety services. Your responsibilities focus on delivering exceptional care, ensuring compliance with regulatory frameworks, and fostering a positive team culture. You will collaborate closely with health professionals and the Senior Manager to implement and evaluate care practices that meet our high standards.
Responsibilities
- Provide high-quality, professional care management advice and assistance to DCR Principals and the Executive Manager (Care Policy and Practice).
- Oversee care management for clients who may be adults with intellectual disabilities, some with a dual diagnosis, challenging behaviours, chronic illnesses, or within the Child Safety portfolio subject to Child Protection Orders and other impacting circumstances.
- Produce effective and clear care plans and practice support frameworks, including individualized training and implementation of care plans.
- Produce clear and factual reporting within required timeframes in line with relevant regulations.
- Ensure that behaviour recording is completed and submitted,
granting support staff clear knowledge of client behaviours.
- Develop care treatment regimens for clients with medical, mental health, and/or complex developmental and behavioural issues in association with other health professionals.
- Conduct regular supervision and training of DCR care staff on-site and prepare official reports and documentation.
- Complete Client Needs Assessments monthly and as required.
- Participate, on a regular rotation, in an after-hours contact roster to provide professional advice to DCR care staff in managing critical incidents and matters affecting client care and support requirements. This includes critical incident reporting for all emergency events.
The Successful Candidate Will Have:
- A completed Bachelor's degree (or higher qualification) in Community Services, Social Work, Human Services, Psychology, Nursing, Child Safety, Youth Work, or a related field—mandatory for this position.
- Minimum 2 years of experience in care/case management.
- Knowledge of legislation and practice in at least one of the following: Disability Care, Forensic Disability, Child Protection, Aged Care, or Health Care.
- Queensland Driver License.
- Must hold or be willing to obtain First Aid & CPR, Blue Card, NDIS Worker Screening Check, and LCS2 Background Check.
