Disability Support Worker (Experienced) – Holistic Care Therapy

About the role
We are seeking an experienced Disability Support Worker to join our team at Holistic Care Therapy, based in Newcastle NSW 2300. This is a casual position that offers flexibility whilst allowing you to make a genuine impact on the lives of people with disability. As a Disability Support Worker, you will provide direct support to individuals in line with their support plans, fostering independence, dignity, and quality of life. This role is central to our mission of delivering person-centred care and plays a vital part in helping our participants achieve their goals and aspirations.
Providing high-quality, person-centred support to people with disability in accordance with their individual goals, preferences, support needs and NDIS plans.
Assisting participants with personal care activities while promoting dignity, privacy, choice and independence. This may include showering, bathing, grooming, dressing, toileting, continence care, oral hygiene, skincare, menstrual care, shaving, hair care, positioning, transfers, mealtime assistance, hydration, and other personal support needs as outlined in the participant's care plan.
Supporting participants with self-care and daily living activities , including meal planning and preparation, household tasks, laundry, cleaning, shopping, budgeting, routine establishment, medication prompts (where appropriate), and developing independent living skills.
Providing safe manual handling , transfers and mobility assistance using approved techniques and equipment, including hoists, standing aids, wheelchairs and other assistive devices, in accordance with participant care plans and organisational policies.
Administering or assisting with medication in accordance with organisational policies,
participant medication management plans, pharmacy instructions and legislative requirements, ensuring accurate documentation of all medication administered.
Reading, understanding and implementing participant care plans, Behaviour Support Plans, Positive Behaviour Support strategies, risk assessments, allied health recommendations and other clinical documentation to ensure supports are delivered consistently, safely and in accordance with best practice.
Supporting participants to engage in community access, recreational activities, education, employment, volunteering and social participation in line with their goals and interests.
Assisting with transport to appointments, therapy sessions, education, employment and community activities while ensuring participant safety at all times.
Encouraging participants to develop confidence, independence and capacity in daily living skills through active support and strengths-based practice.
Maintaining accurate, objective and timely progress notes, communication records, incident reports and other organisational documentation in accordance with company policies.
Monitoring participant health, wellbeing and behaviour, promptly identifying and reporting any changes, risks, incidents or safeguarding concerns to the appropriate person.
Communicating professionally and respectfully with participants, families, carers, Behaviour Support Practitioners, Allied Health Professionals and the broader multidisciplinary team to promote continuity of care.
Building respectful, trusting and professional relationships with participants while upholding their rights, privacy, dignity, cultural identity and individual preferences.
Maintaining a safe, clean and healthy home and community environment by following infection prevention, manual handling, work health and safety and organisational procedures.
Adhering to all organisational policies and procedures,
including duty of care, dignity of risk, mandatory reporting, restrictive practice requirements, privacy and confidentiality, safeguarding, infection control and workplace health and safety.
Participating in mandatory training, supervision, team meetings and professional development to maintain competency and ensure the delivery of safe, high-quality supports.
Working collaboratively with colleagues and the multidisciplinary team to provide consistent, person-centred supports that maximise participant outcomes and quality of life.
What we're looking for
Proven experience working as a Disability Support Worker or in a similar support role, demonstrating substantial knowledge of person-centred approaches
Current working with children's check (WWCC) or disability worker exclusion screening clearance, or willingness to obtain these
First Aid and CPR certification (current or willingness to complete)
Solid communication and interpersonal skills, with the ability to build positive relationships with participants and team members
Ability to work independently and use own initiative, whilst remaining responsive to team direction
Physical capability to provide hands-on support and assistance with personal care
Reliable and punctual, with professional attitudes and strong work ethic
Understanding of disability rights, privacy and confidentiality obligations
Preferred: Experience with participants with specific disabilities such as intellectual disability, acquired brain injury, physical disability or mental health conditions
