About the Role

You'll coordinate care for veterans and Medicare-funded clients living with chronic and complex health conditions. Working via telehealth,



you'll become each client's primary point of contact throughout their care journey, ensuring they receive coordinated, person-centred support.

Key Responsibilities

- Conduct comprehensive biopsychosocial assessments
- Develop and coordinate individualised care plans
- Participate in GP-led multidisciplinary case conferences
- Deliver regular care coordination consultations and 90-day reviews
- Coordinate referrals across allied health services
- Identify and manage psychosocial risks
- Advocate for clients and facilitate access to appropriate services
- Maintain accurate clinical documentation in line with professional standards

About You

To be successful, you'll have:

- Bachelor or Master's degree in Social Work
- Current AASW membership (or eligibility prior to commencement)
- DVA experience desirable
- Strong assessment and care coordination skills
- Excellent communication and rapport-building abilities
- Confidence delivering services via telehealth
- Ability to work independently while collaborating within a clinical team
- Current National Police Check (or willingness to obtain)
