About the role
Right at Home North Queensland is continuing to grow, and we are looking for an experienced and compassionate Care Coordinator/Nurse to join our supportive team based in Charters Towers.
This is an exciting opportunity to play a key role in coordinating high-quality in-home care services across the Charters Towers region. Working closely with clients, families, healthcare professionals and our dedicated care team, you will ensure our clients receive personalised, flexible and outcomes-focused support that enables them to live safely and independently at home.
The successful applicant will ideally have experience in community aged care, disability services or nursing and be passionate about delivering exceptional client outcomes while working in a collaborative team environment.
Key Responsibilities
- Conduct comprehensive client assessments and regular care reviews.
- Develop and maintain government-compliant care plans aligned with Support at Home and client goals.
- Coordinate the delivery of personalised in-home care services.
- Build and maintain positive relationships with clients, families, referrers and healthcare professionals.
- Provide case management and customer service support.
- Deliver direct nursing supports where applicable (EN/RN).
- Assist with personal care services where required.
- Ensure quality assurance, compliance and accurate documentation standards are maintained.
About you:
To be successful within this role, you will bring:
- Diploma of Nursing with current AHPRA registration (non-essential).
- Certificate IV in Ageing Support, Individual Support or a related qualification (minimum)
- Previous experience in community aged care, disability services or home care.
- Sound understanding of the Australian Government aged care funding environment, including Support at Home.
- Experience with case management, care planning and quality and compliance requirements.
- Current Driver Licence and access to a reliable, comprehensively insured vehicle.
- Current National Police Check.
- Current First Aid and CPR Certificate.
- Excellent organisational, communication and time management skills.
- Strong problem-solving abilities and sound clinical or care coordination judgement.
- A genuine commitment to person-centred care and exceptional customer service.
- The ability to work collaboratively within a multidisciplinary team while managing competing priorities.
