About the opportunity: In this rewarding role, you'll partner with older Australians to help them remain living independently in their own homes. Working closely with the Community Aged Care Manager and multidisciplinary teams, you'll coordinate personalised care, oversee client journeys, ensure quality documentation and support compliance with the new Support at Home program and Aged Care Quality Standards.
This is an opportunity to combine your clinical or community services experience with meaningful relationship building, helping clients achieve the best possible outcomes while making a genuine difference every day.
Your day-to-day will involve:
- Managing client referrals, assessments, onboarding and ongoing care planning
- Coordinating services under Support at Home, including care management, short-term pathways and assistive technology or home modifications
- Conducting home visits to monitor client wellbeing, service quality and safety
- Building trusted relationships with clients, families, health professionals and community partners
- Supporting clients to navigate My Aged Care and other government-funded programs
- Maintaining accurate documentation and ensuring compliance with legislation, funding requirements and Aged Care Quality Standards
- Working collaboratively with care teams to deliver safe, person-centred care
- Supporting continuous improvement, quality initiatives and audit readiness
About you:
You'll be passionate about supporting older Australians and enjoy building genuine relationships with clients, families and colleagues. You thrive in a role where no two days are the same and take pride in delivering high-quality, person-centred care.
Additionally;
- Certificate III in Aged Care, Individual Support or Community Care
- Experience delivering or working within Consumer Directed Care or Care management models
- Ability to monitor and support client budget processes
- Knowledge of Strengthened Aged Care Quality Standards,SIRS, Aged Care Act 2025
- Experience using Microsoft Office and care-relatedsoftware
- Current NDIS Worker Screening and a current driver’slicence
