We are partnering with a respected local not-for-profit organisation to recruit a Program Worker – Intake & Assessment to join their Specialist Homelessness Services team based in Tweed Heads .
This is an opportunity to be the first point of support for people experiencing or at risk of homelessness, providing timely assessments, brief interventions and connecting people with the right services before more intensive support is required.
About the Role
Working from the Tweed Heads office, you'll be part of a collaborative team, reporting to an experienced Team Leader within the Specialist Homelessness Services program.
This is a predominantly phone-based role where you'll engage with clients, complete intake and assessment, provide brief interventions, coordinate referrals, and help people access the support they need as quickly as possible.
Key Responsibilities
- Conduct intake and comprehensive client assessments.
- Provide brief intervention and immediate support to individuals and families.
- Assess client needs and determine appropriate referral pathways.
- Connect clients with internal and external support services.
- Maintain accurate case notes and client records.
- Work within a trauma-informed, person-centred framework.
- Collaborate closely with colleagues to ensure positive client outcomes.
About You
- Experience in intake and assessment within community services.
- Applied case management experience.
- An understanding of Specialist Homelessness Services or experience working with people experiencing homelessness.
- A trauma-informed and person-centred approach to supporting clients.
- Strong communication and active listening skills.
- The ability to build rapport quickly over the phone.
- Excellent organisational and documentation skills.
- A genuine passion for helping people navigate challenging circumstances.
