Registered Nurse – Hospital in the Home (Part‑time or Full‑time)
The Service Hospital in the Home (HITH) is Cabrini’s acute outreach service. We provide hospital‑level treatment to patients in the comfort of their homes, as well as care to Post‑Acute Care and chemotherapy patients under our Cabrini at Home program.
Role
We are seeking an experienced Hospital in the Home (HITH) Nurse to join our team on a full‑time or part‑time basis with flexible FTE options. Working across a rotating 7‑day roster, including some PM and on‑call shifts, the nurse will deliver high‑quality acute care to patients in their homes. This senior clinical role involves working both independently and as part of a multidisciplinary team, partnering with physicians, specialists and allied health professionals to provide protected, evidence‑based, patient‑centred care.
Key Responsibilities
- Develop and implement comprehensive care plans tailored to each patient’s needs.
- Monitor patient progress and ensure continuity of care.
- Conduct clinical assessments, administer treatments, and manage patient care.
- Provide education to patients and families on health management and recovery.
About You
- Current AHPRA registration as a Registered Nurse.
- Proven acute and community nursing experience.
- Strong clinical skills in wound care, IV therapy,
and CVC management.
- Ability to work independently and respond to clinical deterioration.
- Excellent communication and documentation skills.
- Commitment to quality improvement and ongoing professional development.
- Valid driver’s licence.
- Working with Children Check.
