About the Role

We are seeking a compassionate and experienced Care Partner with a Registered Nurse (RN) background to join our growing aged care team.

As a Care Partner, you will work closely with older people, their families, service providers, and our internal team to coordinate high-quality, person-centred care. Your nursing background will be particularly valuable in understanding clients with complex health and care needs, identifying potential risks, and supporting appropriate care planning and service coordination.

This role is well suited to an RN who enjoys building relationships with clients and families and is interested in moving into, or continuing in, a care coordination and case management focused role.

Key Responsibilities

- Build strong and trusting relationships with clients and their families.
- Conduct client assessments and identify individual care needs, preferences, goals, and risks.
- Develop, implement, and regularly review individualised care plans.
- Coordinate home care and community-based services to support clients to remain safely and independently at home.
- Apply clinical knowledge when supporting clients with complex or changing health needs.
- Liaise with GPs, allied health professionals, hospitals, support workers, service providers, and family members as required.
- Monitor client wellbeing and respond appropriately to changes in health, function, or circumstances.
- Support medication, clinical care, falls prevention, wound care and other health-related matters within the scope of the role.
- Maintain accurate client records, progress notes, assessments, and care documentation.
- Ensure services are delivered in accordance with the Aged Care Act, strengthened Aged Care Quality Standards, organisational policies, and relevant clinical and safeguarding requirements.
- Advocate for clients and support informed choice, dignity, independence, and quality of life.

About You

To be successful in this role, you will ideally have:

- Qualification as a Registered Nurse,



with current or previous RN experience.
- Current AHPRA registration.
- Fluent Mandarin and/or Cantonese communication skills – essential, particularly for supporting Chinese-speaking clients and families
- Experience in aged care, community care, home care, disability, hospital, or another relevant healthcare setting.
- Strong assessment, care planning, case management, and problem-solving skills.
- Good understanding of the needs of older people, including clients with complex or chronic health conditions.
- Excellent communication and relationship-building skills.
- Ability to communicate effectively with clients from culturally and linguistically diverse backgrounds.
- Strong organisational and time-management skills.
- Confidence working independently while collaborating with a multidisciplinary team.
- Good computer literacy and ability to maintain accurate documentation.
- Current driver's licence and access to a reliable vehicle.
- Relevant police and worker screening checks as required.

Pay: $75,000.00 – $90,000.00 per year
