The Opportunity
An exciting opportunity is available for a Client Coordinator to join our Brightwater At Home Transitional Care Team, which provides short-term, goal-oriented care for 12 weeks that seeks to optimise the functioning and independence of older people after a hospital stay.
This is a full-time, Monday to Friday position, working in the community across the Central and Eastern Metropolitan area and will suit candidates who live within this area.
This role is ideal for someone with experience in health, aged care, or community services who wants to make a real difference in people’s lives and enjoys a fast paced and changing client caseload. It may also appeal to Enrolled or Registered Nurse or Social Worker who are seeking a new direction where their clinical skills are valued and applied in a different way.
The role offers flexible working arrangements, with the option to work from home around your appointments and assessments.

What you’ll bring
- Significant experience in a health, aged or community service workplace with an understanding of Support at Home, CHSP guidelines and Aged Care Quality Standards
- Excellent written, verbal and interpersonal communication skills with the ability to inclusively communicate with a diverse range of people, building and developing purposeful relationships and collaborative teamwork.
- Excellent customer service focus and strong person-centred skills with the ability to effective engage clients requiring services.
- Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
- Working knowledge of occupational safety and health legislation; Aged Care Act (1997); Aged Care Quality Standards (desirable).
- Experience using client management software systems and experience with case management (desirable).
- Clinical/Health Experience i.e Social work/Nursing (desirable).
- A satisfactory National Police Clearance issued within the past 12 months.
- A current valid WA driver’s licence.
