Registered Nurses – Street Outreach, Youth Outreach & Hospital In-Reach

Homeless Healthcare is looking for experienced Registered Nurses to join our Street Outreach, Youth Street Outreach and Royal Perth Hospital (RPH) In-Reach teams. Casual and part time positions are available.

Homeless Healthcare is a dynamic, not-for-profit organisation providing healthcare to people experiencing homelessness or at risk of homelessness across Perth. We run mobile GP clinics and outreach services across a range of settings, including drop-in centres, crisis and transitional accommodation, residential rehabilitation, women's refuges, street outreach and hospital in-reach.

Our outreach nurses work alongside experienced social service professionals to provide flexible, person-centred healthcare to people who may otherwise struggle to access traditional health services.

As a Street Outreach Nurse, you will work alongside social service key workers to provide health assessment, treatment, health education, care coordination and connection to ongoing healthcare. The work is varied and community-based, with no two days looking the same.

Our Youth Street Outreach team works specifically with young people experiencing homelessness or who are otherwise disengaged from mainstream services. Nurses work alongside an HHC Youth Worker and other service partners to provide accessible, trauma-informed healthcare and support young people to engage with appropriate health and social services.

Our RPH In-Reach nurses work with people experiencing homelessness who are admitted to Royal Perth Hospital, supporting them during their hospital admission and helping to coordinate their transition back into the community. The role involves working closely with hospital teams,



patients and community services to support continuity of care beyond the hospital.

Across all of our outreach services, nurses have the opportunity to use their clinical skills in a very different environment, build meaningful relationships with patients and work as part of a multidisciplinary team that looks at the whole person, not just their immediate health needs.

About you

To be successful, you will be a confident and experienced Registered Nurse who is comfortable working autonomously while also being part of a multidisciplinary team. You will have strong communication and interpersonal skills and be able to build rapport with people who may have complex health and social needs or have had difficult experiences with mainstream services.

Essential requirements

- Registered Nurse with current unrestricted AHPRA registration

- Minimum 3 years' post-graduate nursing experience

- Current National Police Clearance

- Current CPR certification

- Full, unrestricted Australian driver's licence

- Proficiency in Microsoft Office 365

Desirable experience and skills

- Experience in mental health and/or drug and alcohol nursing

- Community nursing or Emergency Department experience

- Competence using clinical software

- Experience and confidence in wound care, ECGs, phlebotomy and immunisations

- Spirometry experience

Experience working with people experiencing homelessness, disadvantage or complex social circumstances will be highly regarded.

Most importantly, you will bring a genuine commitment to providing respectful, person-centred care and be comfortable working flexibly in community settings.
