About the Role
As a Care Partner, you'll become the primary point of contact for an allocated portfolio of clients, working closely with clients,
representatives and the wider Clinical Care Team to ensure services remain responsive, coordinated and focused on achieving positive outcomes.
Your responsibilities will include:
Managing an allocated portfolio of clients.
Developing and monitoring care plans.
Completing routine client reviews and follow-up.
Building robust relationships with clients and their representatives.
Identifying changes in client needs and coordinating appropriate responses.
Working collaboratively with the Care Manager and Clinical Care Team Leader.
Supporting high-quality, person-centred care across the organisation.
About You
You'll be someone who enjoys building relationships, communicating with people and working proactively to achieve the best outcomes for your clients.
You'll also have:
Current AHPRA registration as an Enrolled Nurse.
Experience in community aged care, home care or a related clinical setting.
Strong communication and relationship-building skills.
Excellent organisational and time management skills.
Sound clinical assessment skills within your scope of practice.
A commitment to person-centred care and continuous improvement.
