About the Role:
Mana Wellness Disability Services is seeking an experienced and compassionate
Community Support Worker
to join our team on a full-time basis. This role involves supporting people living with disabilities within residential group housing and community settings. The successful applicant will be responsible for delivering person-centred care, promoting independence, supporting daily living activities, and ensuring participants are provided with a safe, respectful, and supportive environment.The role requires an individual who is proactive, resilient, professional, and committed to delivering exceptional outcomes for people living with disability.
Key Responsibilities
Provide high-quality, person-centred support to participants living in Supported Independent Living (SIL) and group housing environments.
Assist participants with personal care, hygiene, dressing, grooming, and meal preparation.
Support participants with medication administration in accordance with care plans and organisational policies.
Assist with household tasks including cleaning, laundry, shopping, and maintaining a safe living setting.
Facilitate community access, social participation, appointments, recreational activities, and skill development.
Implement individual support plans, behaviour support plans, and risk management strategies.
Maintain accurate case notes, incident reports, progress records, and other required documentation.
Support participants with emotional wellbeing, communication, and achieving personal goals.
Work collaboratively with families, guardians, allied health professionals, support coordinators, and multidisciplinary teams.
Respond appropriately to challenging behaviours, emergencies, and crisis situations.
Promote participant choice, dignity, rights, independence, and inclusion at all times
Skill & Experience
Minimum
12 months full time experience
working in disability support, residential care, community services, aged care, mental health, or a related field.
Demonstrated experience supporting people living with disabilities within group housing, residential care, or community-based settings.
Strong understanding of person-centred practice and the NDIS framework.
Experience working with participants who have complex support needs, mental health conditions, psychosocial disabilities,
intellectual disabilities, autism, acquired brain injuries, or challenging behaviours is highly regarded.
Excellent communication, interpersonal, documentation, and problem-solving skills.
Ability to work independently and as part of a multidisciplinary team.
Ability to work a rotating roster including day shifts, afternoon shifts, active nights, sleepovers, weekends, and public holidays.
Essential Qualifications and Requirements
Certificate III in Individual Support, Disability, Community Services, Aged Care, or equivalent qualification (minimum requirement).
Current NDIS Worker Screening Clearance.
Current Working with Children Check (Blue Card).
Current First Aid and CPR Certificate.
Current Australian Driver's Licence.
Ability to meet NDIS Practice Standards and Code of Conduct requirements.
Certificate IV in Disability, Community Services, Mental Health, or related field.
Experience in Supported Independent Living (SIL).
Medication Administration training.
Manual Handling training.
Experience with behaviour support and restrictive practice frameworks.
