About the Opportunity
As an Enrolled Nurse, you will work under the direction and supervision of a Registered Nurse to deliver safe, evidence-based nursing care to patients living with chronic and complex health conditions.
This position plays a key role in supporting patients to manage their chronic health conditions while promoting independence, wellbeing and improved health outcomes. In this role you will:
Deliver person-centred nursing care to individuals living with chronic and complex health conditions in the community.
Assess, monitor and document patients' health while administering medications within your scope of practice.
Collaborate with Registered Nurses and the multidisciplinary team to achieve positive patient outcomes.
Empower patients to manage their health through education, support and evidence-based care.

About You
We are looking for someone who has:
Current AHPRA registration as an Enrolled Nurse (Division 2), with an approved qualification for medication administration, including intravenous medications.
Demonstrated clinical experience and nursing skills relevant to chronic disease management or community nursing.
Excellent communication and interpersonal skills, with the ability to build positive relationships with patients, carers and multidisciplinary teams.




Strong organisational skills, with the ability to prioritise workload and deliver safe, high-quality patient care in a dynamic environment.
A commitment to evidence-based practice, continuous improvement and person-centred care, along with a current valid Australian Driver Licence.
