About the Role
We are seeking an experienced
Clinical Coordinator / Registered Nurse
to provide clinical oversight, nursing support and care coordination within an NDIS service setting.
This role is ideal for a nurse with experience in
disability, community nursing, primary health, aged care, acute care or complex care
who is confident working outside of a hospital setting.
You will support NDIS participants with complex health and disability needs, including neurological conditions, epilepsy, medication support, mobility needs, personal care requirements and other high intensity supports. You will also provide clinical guidance,



training and escalation support to support workers to ensure care is delivered safely and in line with NDIS Practice Standards.
This position involves a combination of community visits, participant assessments, documentation, care plan development, staff training and clinical coordination. Some work from home flexibility may be available depending on operational needs.
Key Responsibilities
Deliver and oversee clinical nursing supports for NDIS participants.
Conduct clinical assessments and identify participant health, safety and support needs.
Develop, implement and review health care plans, clinical support plans and risk assessments.
Support complex care management plans for participants with high intensity or changing health needs.
Oversee medication management, medication documentation and related clinical processes.
Provide clinical advice, education, training and escalation support to support workers.
Assist with staff competency checks, clinical observations and mentoring.
Liaise with GPs, hospitals, allied health providers, families, support coordinators and other stakeholders.
Monitor clinical risk, participant wellbeing and changes in health presentation.
Contribute to incident management,



clinical reporting, investigations and quality improvement.
Ensure clinical documentation is accurate, timely and compliant with NDIS and organisational requirements.
Support compliance with NDIS Practice Standards, relevant policies, procedures and clinical governance requirements.
Promote safe, respectful and person-centred care across all participant supports.
Qualifications & Experience
Current
AHPRA registration
as a Registered Nurse (
EN or AIN in Australia with overseas experiences
may be considered)
Experience in
NDIS, disability, community nursing, or complex care
Knowledge of
NDIS Practice Standards
and clinical governance (PREFERRED)
Experience creating, reviewing or implementing care plans, health support plans or clinical risk assessments
Current
First Aid and CPR
NDIS Worker Screening Check
Working with Children Check
National Police Check
Right to work in Australia
Employment Type and Remuneration
FULL-TIME, Casual or Part-time opportunities available
Remuneration in accordance with the Award applicant falls under, classification and pay point dependent on qualifications, experience, and role responsibilities
