Wintringham is seeking a Social Worker to deliver specialised social work support and targeted case management for older people at risk of homelessness. Reporting to the Senior Social Worker, you will work within the Support at Home model providing billable services including complex service navigation, crisis intervention and advocacy.
We are looking for candidates with a Bachelor of Social Work or equivalent and eligibility for AASW, plus knowledge of aged care systems and the CHSP/NDIS
