You will deliver evidence based, consumer centred care while working collaboratively with families, carers and partner services in line with legislation and professional standards.
Your responsibilities will include:Delivering comprehensive mental health assessments, triage and evidence based therapeutic interventions to adults in a community setting.Providing case management within a recovery oriented framework,



supporting consumers to achieve meaningful health and wellbeing outcomes.Working collaboratively within a multidisciplinary team, and liaising effectively with consumers, carers, families and external service providers.Ensuring care delivery complies with relevant legislation, policies and qualified standards, including the NSW Mental Health Act.Participating in crisis intervention, care planning and risk management, maintaining accurate and timely clinical documentation.Contributing to continuous professional development, clinical supervision and service improvement, while upholding NSW Health CORE values.Your contribution as a healthcare professional will have a meaningful and positive impact, which is felt at a community level.
