Our team provides comprehensive health and developmental assessments, care planning and coordination, working closely with a wide range of partners to ensure each child receives the support they need to thrive.About the roleAs a Health Case Manager, you will play a vital role in supporting children and young people, their carers and caseworkers through holistic, trauma-informed health assessment and care planning.

ll be supported by a collaborative and experienced team, working alongside Paediatricians, Nursing, Allied Health, Case Management and Administration staff.
Comprehensive orientation, training and ongoing professional development are provided to help you succeed and grow in your career.About youYou hold a degree in Social Work, Nursing, Occupational Therapy, Physiotherapy, Psychology or Speech Pathology, with eligibility for membership of the relevant professional association or full registration with AHPRA.
You also bring a minimum of three years?
post-graduate experience working with children and young people in a relevant setting.You are confident delivering child- and family-centred care using a holistic, trauma-informed approach.
You?
re a strong communicator who enjoys working collaboratively and building positive relationships.
Your clinical expertise, advocacy skills and commitment to improving outcomes will make a meaningful difference in the lives of children and young people in out-of-home care.Requirements Must hold a degree in Social Work, Nursing, Occupational Therapy, Physiotherapy, Psychology or Speech Pathology or other qualification deemed equivalent by the employer, which provides eligibility or membership of the relevant skilled association/full registration through the Australian Health Practitioner Regulation Agency.Qualifications: Minimum of 3 years post registration experience relevant to working with children and young people in out of home care with a thorough understanding of trauma informed and culturally safe practice.
