If you're passionate about mental health, enjoy building meaningful relationships and want to make a genuine impact in your community, we'd love to hear from you.
What you'll be doing
No two days are the same in this role.
Working with NDIS participants living with psychosocial disability, you'll provide recovery-focused coaching and support coordination that empowers people to build capacity, navigate services and move toward greater independence and wellbeing.
You'll work alongside participants, families, carers and service providers to ensure supports are person-centred, coordinated and aligned to each individual's goals.
In this role you'll:

Coordinate services and supports that build capacity and increase community participation.
Work collaboratively with families, carers, clinical teams and external providers.
Advocate for participant choice, control and self-determination.
Monitor progress, review outcomes and adapt supports when needs change.
Maintain accurate case notes and documentation in line with NDIS requirements.
Contribute to the growth of Richmind WA's NDIS services through strong community relationships and stakeholder engagement.
What you'll bring
You are someone who genuinely enjoys connecting with people and supporting them to achieve meaningful outcomes.
You bring an understanding of recovery-oriented practice and are committed to delivering services that are person-centred, strengths-based and trauma informed.
You'll also have:
A Certificate IV in Mental Health (or equivalent) or demonstrated experience supporting people experiencing mental distress.
Experience in Support Coordination, Psychosocial Recovery Coaching or a similar community services role.
Knowledge of NDIS legislation, funding arrangements and service delivery requirements.
Strong communication, relationship-building and problem-solving skills.

The ability to manage competing priorities and work autonomously.
A commitment to inclusion, recovery and human rights-based practice.
To be successful, you'll also require:
National Police Clearance (less than six months old)
Working with Children Check
NDIS Worker Screening Check
Current First Aid and CPR certification

This role is fixed-term, full-time until 30 June ****.
It is working Monday to Friday and based out of our Cannington office.
