Job DescriptionEMPOWERA SUPPORT COORDINATION SERVICES PTY LTD Interview NDIS participants and assess their individual circumstances, support needs, personal goals and the nature and extent of any social, emotional, health, housing or community participation difficulties.
Plan, develop, implement and review person-centred support programs that assist participants to achieve the goals identified in their NDIS plans.
Assist participants to understand and effectively implement their NDIS plans, including explaining available supports, funding arrangements and service options.
Provide participants and their families with information, practical guidance, emotional support and advocacy regarding disability, welfare and community services.
Monitor participant progress, evaluate the effectiveness of support arrangements and prepare progress reports and recommendations where changes are required.
Refer participants to appropriate NDIS providers, allied health professionals, community organisations and government agencies that can provide additional assistance.
Assess the availability and suitability of community resources relating to health, welfare, housing, employment, education, training and social participation.
Liaise with participants, families, carers, community groups, welfare agencies, government bodies, private service providers and allied health professionals to coordinate integrated support services.
Assist participants to develop independent living, decision-making,



self-advocacy and community participation skills through capacity-building support.
Support participants experiencing complex social, emotional or financial circumstances by identifying immediate needs and facilitating access to appropriate services.
Promote awareness of available community programs, disability services and mainstream supports that may assist participants to improve their wellbeing and independence.
Qualifications and Experience Applicants must have: An AQF Diploma, Advanced Diploma, associate degree or higher qualification in Community Services.
At least 3 years of relevant experience that may substitute for the formal qualification.
Applicants should also have: Relevant experience in community services, disability services, welfare support, case management or NDIS support coordination.
Demonstrated ability to assess client needs and coordinate support programs.
Sound knowledge of the NDIS, person-centred practice and community service systems.
Robust communication, advocacy, interpersonal and report-writing skills.
Ability to work independently and manage a varied participant caseload.
Experience liaising with families, community organisations, government agencies and service providers.




Mandatory Requirements Current NDIS Worker Screening Check.
Current National Police Clearance.
Working with Children Check, where required.
Current Australian driver's licence.
First Aid and CPR certification, or willingness to obtain.
Full Australian working rights.
