RetireAustralia is seeking an experienced and compassionate
Case Manager (Clinical) – RN
to join our dedicated Support at Home Services team, supporting residents across our Central Coast NSW communities

The villages that you will be covering are Tarragal Glen, Wood Glen, Forrester's Beach and Glengara
Monday to Friday role with occasional on-call requirements - working from our retirement villages.
A requirement to be successful for this role is a Bachelor of Nursing and current RN Registration with AHPRA
The position
As a Case Manager (Clinical) - RN, you will play a pivotal role in supporting integrated and coordinated resident care and service requirements.
You'll be responsible for pre-entry assessment, move-in support, ongoing assessment, care planning, accessing externally funded care, care delivery and evaluation, referral management, social support, and networking.
You'll also support Care Service Workers and contribute to continuous quality improvement across our villages.
You will be based within the region, travelling between villages and other sites as required.
Reporting to the Care Manager, your key responsibilities will include:
Conducting pre-entry assessments for incoming residents
Providing move-in support to ensure a seamless transition
Conducting ongoing assessments to monitor resident needs and preferences




Developing and implementing individualised care plans
Assisting residents in accessing externally funded care resources
Coordinating and overseeing the delivery of care services
Coordinate aged care assessments (ACAT) via My Aged Care where required
Managing referrals to external healthcare providers and services
Facilitating social support and networking opportunities for residents
Monitoring the work practices of Care Services Workers and providing direction and guidance as required
We're on the lookout for someone with:
RN Nursing Registration with AHPRA (required)
Bachelor of Nursing (or Diploma equivalent)
Minimum 3 years' experience in a similar role
Drivers Licence and Reliable Transport (required)
A current Nationally Coordinated Criminal History Check (or willingness to obtain)
First Aid & CPR Certificate (or ability to obtain)
Prepared to undertake a pre-employment medical assessment including a drug and alcohol test
