At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are looking for a dedicated Physiotherapist to be an essential part of our Transitional Aged Care Program team.
This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with notice in alignment with the applicable Award.
Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
About the Opportunity
This position is responsible for providing quality services as part of a multidisciplinary team to patients/clients of Central Coast Local Health District to ensure patients are well managed and optimal health care needs are met.
The Transitional Aged Care Program is a time limited, goal oriented program and targets people at the end of a hospital episode.
Physiotherapy in TACP is aimed at working with clients to:
• Complete the restorative process
• Optimise functional capacity
• Access and finalise longer term care arrangements that may be required.
In this role you will:
Provide a skilled, evidence based clinical service focused on improving the health and wellbeing of the patient/client.
To be conversant with assessment and evidence based treatments across clinical rotations.
Accurately report, record and maintain patients files after each treatment or service transaction within the required timeframes.
Communicate with the Senior Physiotherapist in their relevant area about any problems or difficulties they may be experiencing.

About You
We are looking for someone who has:
Current registration to practice as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).
Relevant post-graduate experience, preferably in a variety of inpatient, rehabilitation and outpatient and community settings.
Effective interpersonal skills, and clear written and verbal communication, with the ability to communicate across all levels with internal and external stakeholders.
Ability to work as an effective part of a multidisciplinary team.
Demonstrated skills in effective time management and the prioritisation of competing work.
Applicants must have current work rights in Australia or be a permanent resident or citizen.
