The Member Health Coach is responsible for delivering evidence-based and trusted health education and advice designed to improve an individual's capacity to manage their health and wellbeing.
As a Member Health Coach, you will contribute to HBFs purpose through the provision of evidence based and trusted health education and advice designed to improve an individual's capacity to manage their health and wellbeing.
Key Responsibilities:
You will optimise health program initiatives and outcomes by driving awareness and encouraging program uptake from eligible members and referring health professionals.
Deliver HBF's telephone health coaching program to members managing risk factors or a chronic disease to improve an individual's knowledge and capacity to self-manage.
Ensure the effective and efficient delivery of HBF's telephone health coaching program in accordance with program clinical and operational guidelines
Act as a subject matter expert for broader organisation health & wellness initiatives
Proactively seek and promote quality improvement opportunities as they relate to the Health Strategy and Health Programs objectives and activities
This opportunity is being offered as full-time or part-time, with a minimum of 3 days per week.
This role is Perth based and HBF supports hybrid working arrangements.
Let's talk about you:




We're seeking a knowledgeable allied health qualified with experience in providing support to people already diagnosed with a chronic disease or who are at high risk of developing chronic disease.
You will bring with you highly developed customer service skills and a holistic approach when providing individual consultation.
Other key skills we'll be looking for include:
Knowledge of motivational interviewing and driving behavioural change
Ability to work autonomously with capacity to set one's priorities and work to deadlines.
Demonstrated interpersonal skills including demonstrated experience and skills in liaison, negotiation, and conflict resolution.
Experience in management of cardiovascular disease and diabetes will be highly regarded.
We are seeking registered Allied Health Professionals from the following professions:
Dietician
OT
Physiotherapist
Pharmacist
Exercise Physiologist
Registered Nurse
