We are a community based, not-for-profit organisation committed to helping vulnerable individuals, families and young people achieve safe housing, independence and positive life outcomes.
We are seeking a passionate and motivated Youth Homelessness Outreach Worker to join our new Youth Mobile Support Program.
This is a rewarding opportunity to work alongside young people aged 16 years and over who are experiencing homelessness or are at risk of homelessness.
Through outreach support, case management and advocacy, you will help young people access safe housing, build independent living skills and connect with the services they need to achieve positive long-term outcomes.

You'll spend time out in the community meeting with young people, advocating on their behalf, working collaboratively with local services and helping create lasting change.

About the Role
As the Youth Homelessness Outreach Worker you will:




Deliver outreach support and case management to young people experiencing or at risk of homelessness.
Complete intake, assessment and risk screening and develop individual case management plans.
Assist young people to access and maintain safe and sustainable housing.
Build young people's independent living skills.
Advocate and coordinate referrals to housing, education, employment, health and community services.
Maintain accurate client records and reporting requirements, including SHIP.
Build strong partnerships with government and community organisations.
Work within a strengths-based, trauma-informed and person-centred framework.
Essential Requirements
Applicants must hold, or be able to obtain prior to commencement:
Current Queensland Driver Licence.
Current Paid Blue Card (or ability to obtain).
National Police Check.
Current First Aid and CPR Certificates.
Certificate IV in Community Services, Child, Youth and Family Intervention or another relevant qualification, or be currently studying towards one.
Relevant industry experience may be considered in lieu of formal qualifications.
Key Selection Criteria
Please address each of the following selection criteria in your cover letter.

1. Hold, or be currently studying, a minimum Certificate IV in Community Services, Child, Youth and Family Intervention or other relevant qualification.
Relevant industry experience may be considered in lieu of formal qualifications.
2. Demonstrated ability to work collaboratively within the organisation and develop effective working relationships with external agencies, stakeholders and community service providers.
3.



Well-developed interpersonal and communication skills, with the ability to engage and support vulnerable young people, communicate effectively with people from diverse backgrounds and respond appropriately to individuals experiencing crisis.
4. Demonstrated empathy, compassion and a non-judgemental approach when working with young people experiencing homelessness, housing instability or other complex needs.
Ability to de-escalate challenging situations while providing emotional support.
5. Demonstrated ability to work independently and as part of a team, showing initiative, flexibility, adaptability and effective problem-solving skills.
6. Knowledge of youth homelessness, housing pathways, community resources and support services, with the ability to advocate and coordinate appropriate referrals for young people.
7. Sound understanding of case management principles and practices, including assessment, goal setting, support planning, advocacy and documentation, with the ability to provide culturally appropriate and person-centred support.
8. Demonstrated computer literacy, including Microsoft Office applications and electronic client management systems, with the ability to maintain accurate client records and meet contractual reporting requirements.
Experience using the Specialist Homelessness Information Platform (SHIP), or the ability to quickly learn new systems, is highly regarded.
