Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health. Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.
We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.
We are committed to enhancing access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
About the Role
Direct Care Workers provide whole of client support, in order to empower clients to lead fulfilling lives according to their own choices. Providing quality, practical, efficient, and effective services to clients includes domestic assistance. Services may also include support for social connection to community, family, and friends, assisting clients in pursuing their wide variety of hobbies, interests, and passions.
Use of own vehicle is required.
Who are we Looking for?
To be successful in this role, you will have a strong commitment to working from an empowerment perspective, promoting the client’s independence, skill development, and self-advocacy.
To be successful in this role, you must meet the following requirements:
- Current first aid and CPR
- Knowledge of occupational health and safety requirements
- Available for a minimum of 10 hours per week, which may include early mornings, late afternoons. Penalty rates apply as per the award
- Current National Criminal History Check, NDIS Worker Screening Check and valid Working with Children Check
- Current drivers licence and registered vehicle
- Fully vaccinated for COVID-19 and influenza
