Community Solutions Group is seeking an NDIS Support Coordinator in Rockhampton to help participants achieve greater independence by assessing needs and coordinating supports within their NDIS plans. You’ll work closely with participants to maximize plans and address barriers by implementing action plans.
You will build relationships with participants, families and providers, travel to visit clients, and manage service agreements while tracking billable hours and outcomes.
