About MRT Rehab

My Rehab Team (MRT) is a small local Allied Health company with big dreams. We're a team of around 15, spanning Physiotherapists, Occupational Therapists and Care Coordination staff. We believe qualified and personal life interchange, so our team leaders place a big emphasis on work culture and values. We provide services under the Support at Home (Home Care Package) funding stream, as well as private and NDIS funding.

Our Office

Our team works out of The Commons on Gipps Street — not your typical allied health clinic. We have a private office upstairs plus access to everything The Commons offers: table tennis, private phone booths, breakout rooms, meditation sessions, kitchen and bathroom facilities, an open-air garden, a wellness space, a podcast studio and more. It's a safe, supported and energetic environment shared with other local companies and teams.

About the Role





We're looking for a compassionate Care Coordinator to join our team, based full-time out of our Collingwood office. You'll play a crucial role in connecting our clients with the right services — managing client intake, invoicing and billing, supporting the client success journey, and coordinating seamlessly across our support network. You'll work alongside other care coordination staff, team leaders and the wider MRT team, staying in daily contact with our Allied Health clinicians to help them deliver a high-quality service. We'll set you up with the technology and resources to hit the ground running.

Key Responsibilities

- Manage client intake and referrals across our platforms (Microsoft, our practice management system and other tools)
- Prepare and manage client files, information and service requirements for our clinical team
- Handle inbound and outbound calls, responding to client, care manager and general allied health queries
- Liaise with multidisciplinary teams to ensure effective communication and seamless service delivery
- Maintain accurate, up-to-date client records in line with relevant legislation and MRT policy
- Support clients and their families with guidance on available services and entitlements
- Build and maintain positive relationships with external agencies, service providers and community partners
- Contribute to continuous improvement of our care coordination processes
- Attend weekly meetings with team leaders and care coordination staff

What We're Looking For

- Experience in customer service and administration




- Strong communication and time management skills, with a growth mindset
- Confidence with Microsoft tools and a willingness to learn new technology
- A background working with funding bodies such as My Aged Care, NDIS or community health services (not essential)
- A happy, positive attitude and genuine enjoyment of working within a team
- Willingness to hold or obtain an NDIS Worker Screening Check and National Police Check
