You will deliver person-centred nursing care in clients' homes, coordinating services and supporting independence and wellbeing.
Requirements include AHPRA registration, 3+ years post-graduate experience in community settings, robust assessment and care planning skills, and excellent communication and teamwork.
A Victorian driver license is required.
