Illawarra Shoalhaven Local Health District invites applications for a Clinical Nurse Specialist (Grade 2) in the Aged Care Outreach Service. The CNS will lead acute care delivery at residents' homes, assess and manage deteriorations, and train RACF staff to recognise and respond to changes.

Based at Wollongong Hospital, you will work with GPs and healthcare teams to ensure seamless care and, when needed, arrange direct hospital admissions while maintaining patient comfort and safety in familiar
