Your opportunity

Looking to start your career in a role that makes a genuine difference? As a Care Partner Support Officer, you'll gain hands-on experience supporting older people and their families while developing your skills within a collaborative and supportive team workplace.

You'll assist with client coordination, caseload administration and service delivery activities, while working closely with experienced professionals who are passionate about helping clients achieve their goals. As a key contact for clients, families and advocates, you'll build confidence, develop your professional practice, and help deliver care aligned with ECH's Wellness Model of Care.

You’ll make an impact by

- Building meaningful relationships with clients, families and advocates as a trusted point of contact.
- Supporting older people to achieve their goals through client assessments, care planning and wellness-focused support.
- Coordinating services and responding proactively as client needs change.
- Working alongside experienced Care Partners to develop your skills in care coordination,



service planning and client support.
- Maintaining accurate client records and contributing to quality service delivery.
- Collaborating with a multidisciplinary team to create positive outcomes and experiences for clients.

This role is ideal for someone who is adaptable, compassionate and keen to develop their career while supporting older people to live well, maintain independence and get more out of life.

What you’ll bring To succeed in this role, you’ll need the following qualifications:

- Recent tertiary qualifications in Human or Behavioural Sciences, Social Work, Allied Health, Aged Care, Community Services or a related field
- Current National Police Check (or willingness to obtain)
- Current driver's licence
