Role Overview
FMHSS operates under the Department of Social Services guidelines, promoting early intervention, trauma-informed practice, family engagement, collaboration with community services, and strengths-based support for children, young people, and families experiencing or at risk of mental health challenges.
We are seeking a dedicated Support Facilitator to join our team. In this role, you will provide short-term case management and deliver psycho-educational programs for children and young people aged 6–18 years. You will also work closely with families to strengthen their capacity to manage emotional and social challenges, while fostering positive outcomes through a collaborative, family-centred, and strengths-based approach.
Key Responsibilities
- Provide program participants with short-term case management in line with FMHSS guidelines.
- Facilitate psycho-educational programs for children and young people aged 6–18 years.
- Deliver ongoing support to individuals, families, and groups to help manage emotional and social challenges.
- Maintain accurate records, data collation, and reporting in compliance with organisational and contractual standards.
- Build Centacare NENW’s capacity through partnerships with government agencies, community organisations, and other stakeholders.
Skills & Experience We’re Looking For
Essential Criteria:
- Relevant qualifications in mental health wellbeing or related fields and/or extensive experience working with young people and their families with complex needs.
- Experience in case management and program delivery, including development, implementation, and evaluation.
- Ability to engage with complex families and deliver effective support.
- Experience working with Aboriginal and CALD communities.
- Strong relationship-building skills with multi-faceted services.
- Current driver’s licence and ability to travel regularly (including overnight stays).
