About the role

We are seeking a reliable and compassionate Disability Support Workers to provide high-quality, person-centered support two new NDIS clients located in the Montrose VIC and or Launching Place VIC. We are looking for support workers that are experienced & comfortable supporting clients with physical disabilities (NDIS participants) and willing to work across all shifts for 24 hour care (applicable penalty rates will apply.) Please note all relevant training and house induction sessions will occur prior to commencing active shifts with the clients.

Main responsibilities and activities may include

- Provide a high quality and timely care support to the clients as directed by Home Caring,



and in accordance with their care plans.
- Support clients in getting out for social engagement, activities and in attending appointments.
- Provide personal care and daily living (domestic) assistance.
- Assistance with household tasks (may include cleaning, meal preparation.)
- Document service activities that support service outcomes.
- Support participant in achieving individual NDIS goals.
- Maintain accurate documentation (including reporting) and shift notes.

Requirements:

- Skilled experience working with clients with disability (experience with participants with complexity highly regarded)
- Excellent communication skills, commitment to a person-centred focus and flexibility to support needs based on individual circumstances
- NDIS Worker Screening Check
- Current First Aid certificate and current CPR certificate
- Current Drivers Licence & have your own reliable vehicle
- The right to work in Australia
