Support Point is growing, and we are looking for experienced Independent NDIS Support Coordinators to join our expanding team.

We are a Registered NDIS Provider delivering Support Coordination across Australia, including metropolitan, regional and remote communities.

This opportunity is ideal for Support Coordinators who want the flexibility of independent contracting while still having access to the systems, administration and support of an established organisation.

About the Role

You will support NDIS participants to understand and implement their plans, connect with appropriate services and work towards their individual goals.

Your responsibilities will include

- Supporting participants to understand and use their NDIS plans
- Connecting participants with suitable providers and community supports
- Monitoring budgets and plan utilisation
- Coordinating with families, providers,



allied health professionals and the NDIA
- Attending stakeholder meetings
- Preparing progress reports, case notes and other documentation
- Supporting plan reassessments and Changes in Circumstances
- Identifying risks and gaps in supports
- Promoting participant choice, control and independence

About You

You will ideally have experience in NDIS Support Coordination or a related community services role, solid communication and organisational skills, and confidence managing your own caseload.

Experience supporting participants with complex needs, psychosocial disability, regional or remote living, or Aboriginal and Torres Strait Islander backgrounds will be highly regarded.

Contractors will require an ABN, NDIS Worker Screening Check, Police Check, appropriate insurance, driver's licence, own computer/phone and Australian working rights.
