About the Role
As a Support Coordinator, you will play a vital role in supporting NDIS participants with complex needs to navigate and maximise their plans. You will work closely with participants, their families, and support networks to coordinate multiple supports, resolve barriers, and implement strategies to achieve sustainable outcomes.
Key Responsibilities
- Coordinate and implement NDIS plans for participants with complex needs.
- Collaborate with participants, families, carers, and service providers to identify goals and develop tailored support plans.
- Navigate and resolve barriers to accessing services, ensuring participants achieve optimal outcomes.
- Build and manage relationships with internal and external stakeholders, including healthcare providers and allied health teams.
- Monitor and review NDIS plan progress, providing reports and documentation as required.
- Provide crisis intervention and risk management support when required.
- Ensure all processes align with NDIS Quality and Safeguards Commission standards.
What We’re Looking For
- Diploma of Community Services, Social Sciences, or related disciplines.
- OR
- Candidates with prior experience in disability services who are seeking opportunities for professional development are encouraged to apply.
- We will also consider entry-level candidates who demonstrate a strong willingness to learn and grow within this role.
- Strong knowledge of the NDIS framework and ability to navigate complex systems.
- Excellent communication, problem‑solving, and time management skills.
- Ability to work collaboratively and independently to meet participant goals.
- High‑level written skills for creating reports, plans, and progress updates.
- Current NSW Driver’s License and willingness to travel locally as required.
- Current National Police Check, NDIS Worker Screening Check, and Working with Children Check.
