About the role
We have an exciting opportunity for a Social Support Coordinator to join our Community Health team in the Yarra Valley, based primarily at Healesville Hospital and local outreach sites. Ideal for someone passionate about social connection and community wellbeing, this role will see you design, coordinate and deliver engaging programs and group activities that help people stay connected, independent and thriving in their community.
Working closely with clients, carers and a multidisciplinary Community Health team, you will develop and deliver flexible, person-centred programs under CHSP and HACC-PYP. You’ll be supported to trial new ideas and co-design initiatives with participants helping shape innovative approaches to social support that strengthen connection, wellbeing and independence across the Yarra Valley.
A regular day will see you:
- Planning andfacilitatinggroup‑based social support and community connection programs
- Working with clients toidentifysocial needs,interestsand participation goals
- Collaborating with allied health,nursingand community partners
- Supporting ongoing program review, co‑designand improvement
- Contributing to innovative approaches to social support in Community Health
About you
You thrive on building connections and creating welcoming spaces where people feel valued and supported. With a creative mindset and strong organisational skills, you enjoy delivering programs that make a real difference.
To be successful in this role you will bring with you:
- Certificate III in Aged Care (or equivalent) with 12 months' experience as a Lifestyle Assistant Grade 1, or Certificate IV in Leisure and Health (or equivalent) with 6 months' experience.
- Experience delivering group‑based social,
community or wellbeing programs.
- An understanding of how social connection supports health,independenceand quality of life.
- A creative and self‑motivated approach, with the confidence to run programs independently.
- Strong communicationskills and the ability to build rapport with a wide range of people.
- Confidence in using digital tools (e.g.Microsoft Office) for planning,communicationand documentation.
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in theAustralian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
