Registered Nurse – Aged Care

Part-Time | 3 Days per Week | Cairns Based | $51.26 - $55.11 / hr

Independent Living Support Association (ILSA) is a not-for-profit community organisation providing quality support services to people with disability and older Australians living independently.

ILSA Home Care Services is one of our key service areas, providing unique, personal and quality supports to older Australians receiving Support at Home funding to remain living safely and well in their own homes and communities.

We are seeking an experienced, compassionate and values-driven Registered Nurse to join our Aged Care team in a part-time capacity, working three days per week, with the opportunity to increase to four days per week.

This is a varied, community-based nursing role combining direct clinical care with assessment, care planning, clinical oversight, risk management, staff education and competency assessment. The Registered Nurse will provide hands-on nursing care to clients where required, including wound assessment and management, while also supporting safe and high-quality clinical practice across our Aged Care services.

This position would suit an experienced nurse who enjoys working independently in the community, values collaborative practice and wants to make a meaningful difference in the lives of older people living at home.

About the role The Registered Nurse will work closely with clients, families, Care Partners, support workers and other health professionals to ensure care is safe, appropriate, person-centred and responsive to each client’s changing needs.

Key responsibilities will include

- Providing direct nursing care to clients in their homes, including wound assessment, wound care and ongoing wound management;
- Undertaking comprehensive clinical assessments and reassessments;
- Developing, reviewing and updating clinical care plans, health management plans and risk assessments;
- Monitoring clients’ health status and identifying changes in their health, functioning or clinical needs;
- Responding to clinical deterioration and coordinating appropriate treatment, escalation, referral or follow-up;
- Providing nursing support relating to medication management, continence, skin integrity, falls, chronic health conditions and other clinical care needs;
- Liaising with general practitioners, hospitals, pharmacists, allied health professionals, families and other service providers;




- Providing clinical guidance and support to Care Partners and support workers;
- Supporting the safe implementation of clinical and high-intensity care requirements;
- Conducting support worker training, competency assessments and clinical supervision;
- Reviewing clinical documentation, progress notes, incident reports and emerging risks;
- Contributing to clinical governance, quality improvement, audits and policy development;
- Promoting dignity of risk, informed choice and person-centred care while ensuring clinical risks are appropriately identified and managed;
- Supporting the organisation to meet its aged care quality, safety and regulatory obligations.

The role will primarily support clients in Cairns, although occasional travel to visit clients on the Atherton Tablelands may be required.

About you

We are looking for a Registered Nurse who is:

- Currently registered with the Australian Health Practitioner Regulation Agency as a Registered Nurse;
- Experienced in aged care, community nursing, home care or a related clinical setting;
- Confident providing nursing care independently in clients’ homes;
- Experienced in wound assessment, wound care and wound management;
- Skilled in undertaking clinical assessments and developing practical, person-centred care plans;
- Experienced in identifying and responding to clinical deterioration, health risks and changing care needs;
- Knowledgeable about medication management, continence, skin integrity, falls prevention, chronic disease and other common clinical needs affecting older people;
- Able to balance client choice and dignity of risk with sound clinical judgement and duty of care;
- Comfortable providing clinical advice, education, supervision and competency assessment to support workers;
- A strong communicator who can build respectful relationships with clients, families, workers and health professionals;
- Organised, responsive and able to work independently while remaining part of a collaborative team;
- Practical,



approachable and committed to safe, respectful and high-quality care;

A current driver’s licence and the ability to travel locally are required. Benefits of working with ILSA The position will be remunerated under the Nurses Award 2020 at Registered Nurse Level 2 or Level 3, depending on the successful applicant’s experience and qualifications.
