Position Overview:
1. As a Nurse – Support at Home, you will support the coordination and review of care for older people living independently at home.
2. This role is ideal for a Registered Nurse or Enrolled Nurse who wants to build a career in community aged care, care planning and client relationship management.
3. You do not need previous Support at Home or Home Care Package experience. Training will be provided in care planning, documentation, client reviews, risk identification and community aged care operations.
4. You will work closely with clients, families, representatives, support workers, allied health professionals and our internal clinical team to help deliver safe, person-centred care.
5. This role allows nurses to continue using their clinical skills in a community setting through client assessments, care plan reviews, risk identification, escalation of clinical concerns, health and wellbeing monitoring, and collaboration with allied health and other care professionals. This role combines nursing knowledge, client communication, care planning and service coordination. It is suited to someone who wants a Monday to Friday role while still using their clinical skills every day.
Who We’re Looking For:
We are seeking a Registered Nurse or Enrolled Nurse who is clinically minded, organised,
warm with people and genuinely interested in helping older Australians live safely and independently at home.
Junior RNs and ENs are strongly encouraged to apply.
You do not need to know everything on day one. If you bring the right attitude, communication skills and willingness to learn, we will provide the training and support to help you grow into the role.
This opportunity is ideal for someone who enjoys building relationships with clients and families, coordinating care with multidisciplinary teams and helping older people receive the right support before small issues become bigger problems.
Allied Health Professionals with strong community aged care or care management experience may also be considered.
Desirable Qualifications:
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Experience in aged care, home care, community nursing, disability, rehabilitation, primary health or hospital nursing
- Experience in case management, care management, Home Care Packages, CHSP or Support at Home will be highly regarded
- Allied Health qualifications with relevant community care experience may also be considered
- Ambulance, paramedic or emergency care experience will be highly regarded
- Current driver’s licence
- Strong computer, documentation and communication skills
What You’ll Do:
- Work closely with clients, families and representatives to support safe, personalised care at home
- Assist with client assessments,
reviews and care planning
- Identify changes in a client’s health, wellbeing, mobility, cognition or support needs
- Escalate clinical concerns appropriately and support timely responses
- Develop and review care plans that promote dignity, choice, independence and reablement
- Communicate with support workers, allied health professionals and external service providers
- Support a diverse caseload of older clients with a range of care needs
- Maintain accurate documentation, client notes and care records
- Contribute to quality, compliance and continuous improvement
- Help clients remain living safely and confidently at home for longer
