Job Description

- Key Responsibilities Provide personal care support, including assistance with showering, dressing, grooming, and maintaining personal hygiene. Assist the participant with getting out of bed and support daily routines to promote comfort, dignity, and independence.
- Offer social support by facilitating transportation to social activities, shopping, community outings, and medical appointments. Perform light domestic assistance, including general cleaning, laundry, and meal preparation, to maintain a secure and comfortable home environment.



Support participants with complex or challenging needs while delivering person-centred care in accordance with individual care plans.
- Community transportation. Maintain accurate records and provide timely updates on participant progress and any significant changes following each service. Promote participant choice, independence, dignity, and community participation while ensuring their health, safety, and wellbeing at all times.
