As a Supportive Care Case Manager, you'll support people and families impacted by blood cancer through compassionate, personalised care, via phone, video, face-to-face and digital channels.
It's a Monday to Friday role with flexible hours, no nights, weekends or on-call, and the freedom to plan your week across office-based work and hospital visits, backed by onsite training and ongoing CPD.
About the role
Reporting to the Clinical Services Manager, you'll manage a caseload of clients, patients, carers and their loved ones, delivering personalised, high-quality supportive care from pre-diagnosis through to survivorship and end of life. This includes comprehensive assessments, care planning, and regular outreach visits alongside phone, video and digital support.
You’ll also build robust relationships with hospitals and health services,
keeping the Leukaemia Foundation an accessible, visible presence in your region.
What you'll be doing
- Managing a caseload of clients, providing tailored emotional, practical and information support
- Delivering high-quality supportive care through regular outreach visits and face-to-face support
- Travelling regularly to regional Victorian hospitals to provide outreach support and build health service partnerships
- Conducting comprehensive assessments and developing personalised care plans
- Supporting clients across the continuum of care, including grief and loss
- Facilitating connections with internal and external services to enhance social support
- Collaborating with healthcare professionals to ensure coordinated, holistic care
- Maintaining accurate healthcare records in line with policy and regulatory standards
- Monitoring and evaluating the effectiveness of care plans
- Contributing to quality improvement initiatives within the Health Services team
What you’ll bring
- Tertiary qualification in social work, occupational therapy, physiotherapy, nursing, psychology or dietetics
- AHPRA registration or eligibility for professional body membership
- Strong understanding of blood cancer and its impact on patients and families
- Knowledge of supportive care services and resources for blood cancer patients
- Compassionate, empathetic communication skills across face-to-face, phone and digital channels
- Strong organisational and time management skills
- Confident with IT systems and maintaining accurate patient records
- Comfortable working independently and within a multidisciplinary team
- Emotional resilience to handle sensitive and distressing situations

- Standard probity checks apply
- You’ll need to provide evidence of vaccination or immunity against measles, mumps, rubella and varicella, and vaccination against pertussis
- A current and valid Working with Children Check (WWCC), held prior to commencement and maintained for the duration of employment
- Ability to travel domestically if required
