About the opportunity:
In this rewarding role, you'll partner with older Australians tohelp them remain living independently in their own homes. Workingclosely with the Community Aged Care Manager and multidisciplinaryteams, you'll coordinate personalised care, oversee clientjourneys, ensure quality documentation and support compliance withthe new Support at Home program and Aged Care QualityStandards.

About you:

You'll be passionate about supporting older Australians andenjoy building genuine relationships with clients, families andcolleagues. You thrive in a role where no two days are the same andtake pride in delivering high-quality,



person-centred care.

Additionally;

- Certificate III in Aged Care, Individual Support orCommunity Care
- Experience delivering or working within Consumer DirectedCare or Care management models
- Ability to monitor and support client budget processes
- Knowledge of Strengthened Aged Care Quality Standards,SIRS, Aged Care Act 2025
- Experience using Microsoft Office and care-relatedsoftware
- Current NDIS Worker Screening and a current driver’slicence
