Community Care Team is seeking a Support Coordinator to assist NDIS participants with complex needs to navigate and maximise their plans. You will work with participants, families, and providers to coordinate supports, resolve barriers, and implement strategies for sustainable outcomes.
The role requires knowledge of the NDIS framework, robust communication and time management skills, and a willingness to travel locally in NSW. NSW Driver’s Licence and relevant checks are required.
