Job Description

We are seeking a passionate, driven Home Care Partner, to join our North team working closely with the Care Team Partners and Assistant Operations Managers. In this rewarding role, you'll deliver safe, effective, and individualised care management for Home Care clients, promoting independence and building strong relationships with internal and external stakeholders to plan and provide high-quality, person-centred support., Mercy Care values individuality. About the Candidate

You're a compassionate and confident Home Care Partner with a deep understanding of aged care services and a strong commitment to person-centred care. You bring a collaborative and empathetic approach to your work,



building meaningful relationships with clients, families, and colleagues. Your supportive and inclusive leadership style empowers older Australians to live well and independently in their own homes.

In addition, you will also possess:

- Certificate IV in Ageing or equivalent
- Experience in community, aged care or support coordination, with a solid understanding of person-centred care.
- Strong organizational and time management skills
- Robust understanding of aged care standards and compliance
- Current first aid certificate or willingness to obtain
- Drivers Licence
- National Police Clearance (obtained within the last 6 months)
