We are seeking a compassionate and organized Care Coordinator to join our team. In this role, you will coordinate care services, support clients and their families, and work closely with healthcare professionals to ensure high-quality, person-centered care.
Key Responsibilities
Coordinate and manage care plans for clients.
Conduct client assessments and monitor ongoing care needs.
Liaise with clients, families, support workers, and healthcare providers.
Schedule and coordinate care services.
Maintain accurate client records and documentation.
Ensure compliance with relevant policies, procedures, and quality standards.
Respond to client enquiries and resolve care-related concerns.
Support continuous improvement initiatives to enhance client outcomes.
Requirements
Experience in care coordination, case management, or community services.
Excellent communication and interpersonal skills.
Strong organizational and time management abilities.
Ability to work independently and collaboratively.
Proficiency in Microsoft Office and client management systems.
Relevant qualifications in Health, Community Services, Nursing, or a related field are preferred.
Current driver's licence and relevant clearances (where required).
Salary
Monthly Salary: AUD $6,000–$7,500, depending on qualifications and experience, plus applicable benefits.
