Key Responsibilities
Manage a growing Support at Home portfolio, whilst ensuring care options align with approved service list.
Act as the first point of contact for potential clients, providing guidance on in-home care options.
Coordinate client onboarding, including care planning, agreements, individual budgets,



service scheduling, and documentation.
Monitor client health outcomes and collaborate with internal teams on care adjustments.
Provide support, ensuring care aligns with industry standards and internal policies.
Participate in on-call support as rostered and liaise with external providers for holistic care delivery.

Desired Skills and Experience
Certificate III or IV in Aged Care (Desirable)
Experience in clinical case management, assessment, and care planning (Desirable)
Robust communication skills both verbal and written
Understanding of Aged Care Quality Standards and relevant legislation (Desirable)
Compassionate, organised, and committed to continuous improvement
Current NSW Driver’s Licence and insured vehicle
