Caladenia Dementia Care is seeking a Program Coordinator to lead the client journey from enquiry to ongoing review in Melbourne's eastern suburbs.
You will coordinate intake, assessments, and Support Plans for people living with dementia and their carers, liaising with My Aged Care and health professionals to ensure responsive, person-centred care.
You will contribute to program development, quality improvement and staff training while maintaining accurate records and aligning with aged care
