Provide personal care, including showering, dressing, grooming and toileting when required
• Support clients with daily living activities and household routines
• Assist with meal preparation and maintaining a clean and safe home setting
• Support clients with community access, appointments, shopping and social activities
• Provide medication assistance in accordance with training, policies and individual support plans
• Follow individual care plans, behaviour support plans and risk-management strategies
• Complete progress notes, incident reports and other required documentation accurately
• Observe and report changes in a client’s health, behaviour or wellbeing
• Maintain clients’ privacy, dignity, confidentiality and personal choices
• Encourage independence and participation rather than doing everything for the client
• Communicate professionally with clients, families, colleagues and management
• Work cooperatively as part of the Group Home team
• Follow SPWD policies, procedures and NDIS requirements
• Attend training and team meetings when required
• Complete morning, evening and sleepover duties according to the roster
