MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. We are currently seeking compassionate and dedicated Care Partnerto join our team based in the Melbourne, Australia.

Responsibilities
Demonstrated ability to build strong partnership relationships with client
Provide comprehensive case management for each client




Build strong Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client's budget
Re-assess client care plans, goals, OHS assessment, and emergency plans for clients
Maintain solid client notes
Nimble in their ability to support clients
About You
Ability to speak Greek language is required
Proficiency in computer skills
Excellent communication and interpersonal skills
Ability to work independently and as a part of a multidisciplinary team
