Access Support Coordination – Rockhampton QLD
Behind every great NDIS journey is someone who made it possible. That someone could be you.
As part of the Access Support Coordination team in Rockhampton, you'll be the person participants lean on when navigating the NDIS feels overwhelming. You'll help them understand their options, make informed decisions and get every bit of value out of their funding — all while building the kind of trust that makes a real difference in someone's life.
It's meaningful, sometimes complex and always rewarding work. If you're someone who shows up with heart, genuinely invests in the people you support and finds purpose in helping others move forward — this is the role you've been looking for!
What You’ll Be Doing
- Supporting participants to build meaningful, connected lives beyond their funded supports
- Helping participants understand and get the most out of their NDIS plans
- Coordinating supports efficiently to maximise outcomes and funding utilisation
- Gathering supporting evidence that reflects participants' evolving needs and future funding requirements
- Identifying opportunities across employment, recreation, education and housing
- Advocating for participants and liaising with mainstream services to ensure the right supports are in place
