Hunter Primary Care is recruiting a Service Mapping & Intake Officer to sit at the heart of the Spoke service.
You will triage referrals, build a regional directory of services, and connect clients across the Hunter, Recent England and Central Coast with the right care quickly, safely and seamlessly.You will work with our Telephony Intake Hub to ensure referrals are matched to appropriate levels of care and coordinate with GPs and allied health providers.
