Team members include a social worker, physiotherapist, dietitian, registered and enrolled nurses and a Clinical Nurse Coordinator.To perform this role,



you will have broad based clinical experience in the area of assessment and management of people experiencing complex and chronic health issuesas well as a demonstrated knowledge and understanding of cardiac and respiratory service delivery across the health sector.
Your excellent communication and interpersonal skills, an ability to work collaboratively within an interdisciplinary team and a well-developed understanding of an evidence-based approach to person-centred care, will be well suited to the role.Please Note: This role can be combined with the position currently advertised "Exercise Physiologist/Physiotherapist" with the Chronic Pain ClinicIf combined this would provide the successful candidate with up to full time work across the two programs.
