This permanent full time role offers a rewarding chance to support older adults with mental health challenges in a community setting.You will perform psychosocial assessments, develop recovery and wellbeing plans, coordinate with families, carers and aged care providers, and navigate My Aged Care and ACAS to arrange sustainable community support.
